The UPMANILA The Perspectives of Pharmacy, Medical and Nursing Students Towards Interdisciplinary Approach in Medication Management

The objective of the study was to determine the understanding by pharmacy, medical, and nursing students of Interdisciplinary Approach in medication management. Pharmacy, medicine, and nursing students of the University of the Philippines Manila were asked to answer a questionnaire approved by the Ethics Review Board that employs a six-point Likert scale. The tools included a modified Interdisciplinary Education Perception Scale (IEPS) and a 10-item constructed questionnaire to measure acceptance of the clinical roles of the pharmacist in medication management. IEPS is a psychometrically validated scale to measure interdisciplinary perception based on four factors: Competence and Autonomy, Perceived Need for Cooperation, Perception of Actual Cooperation, and Understanding Others' Value. Results showed that mean total IEPS differed significantly among student groups (p = 0.0001), with medical students scoring the highest. Mean total scores with the constructed scale also differed significantly (p = 0.0001), with pharmacy students scoring the highest. It has been shown that the mean total scores for both did not differ significantly by age, sex, and other characteristics of the sample population, but differed significantly by health course. A weak positive correlation was established between IEPS and constructed questionnaire scores ( = 0.2986, p = 0.0002). Findings suggest that open-mindedness towards interdisciplinary collaboration would not have a considerable influence on one's acceptance of the clinical roles of the pharmacist with regard to medication management, although, some influence, however slight, may be exerted. There is a need for real interdisciplinary collaboration that may increase the acceptance of the clinical roles of the pharmacist.

The study determined the physicians' level of support toward Medication Therapy Management (MTM) services as measured by their attitude toward collaboration and frequency and reasons for communication with pharmacists, and how these measures were influenced by demographics, knowledge about MTM, physicians' expectations of and experiences with clinical pharmacists. A self-administered and pretested questionnaire was given to 90 residents practicing in the selected wards of the Philippine General Hospital. The main sections of the questionnaire consisted of statements on the physicians' knowledge about MTM, expectations of and experiences with clinical pharmacists, and frequency and reasons for communication. With a response rate of 57.9%, majority of the physicians were knowledgeable about the concept of MTM. Physicians had high expectations of pharmacists as drug therapy experts but uncomfortable with pharmacists suggesting over-the-counter (2.4±0.9) and prescription medications (2.5±0.8) to patients. A number of physicians disagreed that pharmacists routinely counsel the patients about cost-effective drugs (52.7%). Forty-nine percent (49.1%) disagreed that pharmacists, in their experience, ask them for clarifications about drug therapy objectives, inform them of drug-related problems, and take responsibility for resolving these problems. The physicians' attitude toward inter-professional collaboration was significantly influenced by their expectations of pharmacists (p=0.0021), whereas, knowledge about MTM had a substantial effect (p=0.0071) on frequency and reasons for communication. The physicians' demographics appeared to have no association with the two measures of physicians' level of support. Physicians acknowledged the role of clinical pharmacists as drug therapy experts and reliable sources of general and clinical drug information. However, it appeared that they were not open to the independent-decision making responsibilities of clinical pharmacists. Nevertheless, despite the finding that physicians never or rarely interacted with pharmacists on drug inquiries, these health care professionals had a positive attitude towards collaborative practice.

The worldwide use of Traditional and Integrative Medicine (TIM) continues to grow tremendously as a vital part of health care. It is important that health care professionals are knowledgeable on the use of TIM by patients since these therapies may be either beneficial to them or adversely interact with the conventional treatment given to the patients. This study described the use of TIM among adult patients. Data were collected using an interviewer-guided questionnaire. The patients’ socio-demographic characteristics, self-reported health status before using TIM, reasons for using TIM, perception and level of satisfaction on TIM use were identified. Majority of the adult TIM users were females and within the age bracket of 50 to 64 years old, with a high family income and educational attainment. The self-reported health status of the majority of the respondents before using TIM was fair. Among the TIM modalities, acupuncture, herbal medicine, Hilot, massage, and pranic healing were the most frequently used. Most of the patients had a positive perception of TIM. The respondents' major reasons for utilizing TIM were safety and ability of TIM to improve their health. Patients were very satisfied with the result of TIM use and the services given by the clinic. The results of the study are relevant to the health care team's better understanding and recognition of patients as they seek, support, and utilize many different ways for the cure and improvement of their health. The study may facilitate the establishment of traditional and integrative medicine as part of the health care system which may lead to a better quality of health.

Communication plays a big role in fulfilling the goals of pharmaceutical care. It is through medication counseling that pharmacists and the public interact to produce better therapeutic outcomes. This service, however, has not yet been fully established in the Philippines and a gap still exists between the two parties. This study determined the societal perspectives of residents of San Andres, Manila on the role of community pharmacists in medication counseling and explore the relationship of demographic variables and previous counseling experience to societal perspectives. A non-experimental, analytic cross-sectional design was employed in the research conducted from November 2010 to February 2011 through an interviewer-administered semi-structured questionnaire. A total of 334 participated in the study. Majority of the respondents were females, aged 40-49 years old, reached high school level, and had a monthly income of less than PHP 5,000. Fifty five percent perceived community pharmacists as drug sellers and few knew about their counseling role. However, 85.6% of the total respondents expressed acceptance on the possible incorporation of the service in the community setting. Among the different variables that were found to be significantly associated with societal perspectives were sex, age, educational attainment, social class, and counseling experience.

The research determined the wound healing action of an ointment formulation from Pleurotus florida (Mont) Sacc (Fam Tricholomataceae) and performed cost analysis on the product. Fresh fruiting bodies of the fungal material (mushroom) were extracted with 70% ethanol. Several tests were conducted separately on the hydroalcoholic extract of P florida and on the formulated Fleur ointment. Phytochemical screening of the mushroom extract revealed the presence of tannins and glycosides. The extract was found to be stable, whether sealed or unsealed, with or without light, at 5°, 28°, and 40°C. The ointment consisted of white petrolatum, beeswax, and the P florida extract and was prepared at concentrations of 5, 10, 15, and 20%. The effect of 5% ointment on wound healing was comparable to the commercially available wound healing ointment,
Solcoseryl , in terms of the percentage of wound contraction and period of epithelialization performed on Sprague-Dawley rats. There was no significant difference in the effect of the various concentrations of Fleur ointment on the percentage of scar formed. Fleur ointment was a homogeneous light brown mixture that was easily spreadable and smooth. It was stable at 28°C but unstable at 40°C. Based on the cost analysis done on Fleur ointment, the 15-gram 5%
ointment formulation was more affordable than the commercially-available Solcoseryl. The study concluded that Fleur ointment was a low cost and effective agent for wound healing.

Diabetes mellitus is one of the leading causes of mortality in the Philippines. Several formulated anti-diabetic drugs, such as Metformin, have already been studied and established. However, since these drugs are expensive, some people resort to cheaper alternatives, like natural anti-diabetic substances to lower their blood sugar or help prevent diabetes. Folkloric use of Ipomoea batatas L. leaves as anti-diabetic has been recorded in other parts of the world. The study determined the interaction between Ipomoea batatas L. leaf extract and metformin in terms of anti-diabetic activity. Ipomoea batatas L. (Fam. Convolvulaceae) has been used and well-studied for the treatment of diabetes. Metformin, a biguanide, is a common anti-diabetic agent used in the Philippines and is the first line therapy for Type 2 diabetes. Even though metformin may interact with some drugs and cause drug-herb interactions, there are still no known drug-herb interactions between metformin and Ipomoea batatas L. The study employed a pretest-post test design. Metformin, at a dose of 100mg/kg, the ethanolic extract of Ipomoea batatas L. leaves, at a dose of 100mg/kg, and the combination at a dose of 50 mg/kg and 100mg/kg of each drug was administered to alloxan-induced diabetic Swiss mice. The resulting decrease in the blood glucose levels was measured with a glucometer using the blood sample collected from the
saphenous vein of the mice. The blood samples were collected after the 9 day of administration of the drugs. Results showed a significant reduction in the blood glucose levels after the combination compared to the substances administered alone. In conclusion, there was an additive effect observed in the concomitant administration of the two substances. It is recommended to consider Ipomoea batatas L. (Fam. Convolvulaceae) in the development of a formulation for the management of Type 2 diabetes.
